Vaccine Requirements

Dear Shelton School District Families,

Now more than ever we are focusing on keeping students healthy and safe. This includes keeping students safe from vaccine-preventable diseases. The Washington State Department of Health and Office of Superintendent of Public Instruction govern vaccine requirements. Per these requirements, only students who are fully immunized (this includes actively becoming compliant with vaccinations) and those with medically verifiable documentation of vaccination or exemptions, will be allowed to come to school for face-to-face instruction. State laws and rules, as well as district policy, require compliance with immunization requirements on or before the first day of attendance at school. The purpose of the state immunization requirement is the same as the goal for the Shelton School District: to protect the health of the public and individuals against certain vaccine-preventable diseases (DOH, August 2020) and to provide safe learning environments to all students and staff. Students who are behind on required immunizations will need to comply before scheduled in-person learning begins. Please understand that the vaccine requirements for students were implemented before the COVID-19 pandemic occurred.

Medical, Religious, and Personal Exemptions, signed by a licensed health care provider are acceptable for meeting school entrance requirements. However, Personal Exemptions are no longer allowed for MMR (Measles, Mumps, Rubella) vaccinations. If a signature is missing from a licensed health care provider, you will be asked to obtain the signature before the student's entrance to school. The only exception to obtaining a licensed health care provider’s signature is a religious exemption for individuals who have never been to a doctor. All exemption documents are available at your health care provider’s office. They cannot be provided by the school.

Only medically verifiable vaccination documentation is approved as proof of immunization. Medically verifiable documents include the Certificate of Immunization Status (CIS) printed from any state Immunization Information System (IIS), in Washington that is the WAIIS; any printed document from a clinic; any handwritten documentation with clinic name or stamp; lifetime records with clinic initials and name.  Families can access their immunization records on the WAIIS by signing up at Once your student receives their vaccinations, please provide a medically verifiable copy to the school so we can ensure your student’s immunization record is up to date.

Any student who is homeless or in foster care is allowed entrance to school without full vaccination compliance, but must be actively pursuing vaccination requirements during the school year. Please speak with our McKinney-Vento Liaison to see if you qualify under this provision: Betty Uriostegui (360) 462-2215.

Obtaining required vaccinations:

  • If your student has medical insurance, you can find vaccination providers covered by your insurance on the insurance website or calling your insurance customer service. Then you must call the clinic and make an appointment for vaccinations.
  • If your student does not have medical insurance, you can contact Mason Health: Health Plan Finder Susana Lopez at (360)432-7766. She can also help you find a clinic for vaccinations.
  • Local pharmacies may also provide vaccinations for children, with medical insurance. You should call the pharmacy before you go to ensure they have vaccinations available and that they take your medical insurance.
  • Shelton Pharmacy may also be able to provide vaccinations to children who either have or do not have medical insurance; you must call ahead to ensure the vaccination is available. They participate in the Vaccines for Children program providing free vaccinations to children who do not have medical insurance. They are located at 110 W. K St, Shelton, WA. Their phone number is (360)545-4333.

If you have any questions about vaccination compliance, how to obtain necessary vaccinations, or general vaccination requirements, please contact one of the District Nurses at the numbers below.
