Immunization Information
Welcome to the Shelton School District. Here is information you need to know before your student starts school regarding immunizations, medications and life-threatening health conditions.
Immunizations
Before your student is allowed to come to school they need to be fully vaccinated against vaccine preventable diseases, and have medically verified immunization records, a printed Certificate of Immunization Status (CIS), or health care provider signed Certificates of Exemption at school as proof. Families can have access to their immunization records by signing up on WA.MyIR.net- directions are on an enclosed flyer. If your student is not fully vaccinated or does not have medically verified proof of vaccinations they will not be allowed to come to school for in-person learning, however they may still attend on-line/distant learning without being fully vaccinated. Please get them vaccinated before school starts, or before they come to in-person learning.
Medications
If your student needs medication at school, there must be a physician signed medication order at school. The Authorization for Medication Administration at School form is available on the Shelton School District website on the Health Services page. Your health care clinic can fax this completed form to school, or you can bring it to school directly. All medications must be hand-delivered by an adult, not by the student. Life-Threatening Health Conditions
If your student has a life-threatening health condition, they must have an emergency care plan and any necessary medications and orders at the school before they can attend school. Per district policy, if the school does not have a medical plan for a student with a life-threatening health condition, the student will not be allowed to attend school. For assistance with this, please contact your School Nurse before school begins.
